Open vs. Closed Rhinoplasty: What’s the Difference and Which Approach Is Better?

When patients begin researching rhinoplasty, one of the first technical distinctions they encounter is the difference between open and closed rhinoplasty. Both approaches accomplish the same fundamental goal—reshaping the nose—but they differ in how the surgeon accesses the nasal structures to do so. Double board-certified facial plastic and reconstructive surgeon Dr. Eric Yavrouian, serving Pasadena, Glendale, and Burbank, CA, is experienced in both approaches and selects the technique best suited to each patient’s specific anatomy and goals.

What Is Closed Rhinoplasty?

In a closed rhinoplasty (also called endonasal rhinoplasty), all incisions are made inside the nostrils. There is no external incision on the skin of the nose, which means no visible external scar. The surgeon works through the nostrils to access the underlying cartilage and bone structures.

Advantages of Closed Rhinoplasty

  • No visible external scar
  • Less post-operative swelling in many cases—the nasal skin is not lifted off the underlying structure
  • Faster initial recovery in some patients
  • Appropriate for patients requiring more limited changes

Limitations of Closed Rhinoplasty

  • Limited visualization—the surgeon cannot see the entire nasal structure simultaneously
  • More technically demanding for complex maneuvers
  • Less precise placement of grafts and sutures
  • Generally less appropriate for significant tip work, complex revision cases, or cases requiring substantial structural grafting

What Is Open Rhinoplasty?

In an open rhinoplasty (also called external rhinoplasty), the surgeon makes a small incision across the columella—the narrow strip of skin between the nostrils at the base of the nose—in addition to incisions inside the nostrils. This allows the nasal skin to be folded back, giving the surgeon complete, direct visualization of all the nasal cartilages, bones, and structures simultaneously.

Advantages of Open Rhinoplasty

  • Complete visualization of the nasal anatomy—the surgeon sees exactly what they are working with
  • Greater precision in cartilage reshaping, suture placement, and graft positioning
  • Better suited for complex cases including tip refinement, revision rhinoplasty, and structural grafting
  • More reproducible, predictable results

Limitations of Open Rhinoplasty

  • A small scar on the columella—though this typically heals to near-invisibility within several months
  • Slightly more post-operative swelling, particularly in the nasal tip, due to the lifting of the skin envelope
  • Marginally longer initial recovery in some patients

Which Approach Is Better?

There is no universally superior approach—the best technique is the one most appropriate for each individual patient’s anatomy and the complexity of the changes being made. That said, the majority of experienced rhinoplasty surgeons today use the open approach for most cases because the visualization advantages translate directly into more precise, predictable, and reproducible results. The columellar scar of an open rhinoplasty—which sits in a relatively inconspicuous location—heals exceptionally well in most patients and is rarely a meaningful cosmetic concern after the first few months.

Closed rhinoplasty remains the preferred approach for certain patients—those requiring very limited changes, those with anatomy that lends itself to the endonasal approach, or cases where minimizing post-operative swelling is a priority. Dr. Yavrouian will recommend the approach most appropriate for your specific goals and anatomy during the consultation.

What Really Determines the Outcome

The more important variable is not open versus closed—it is the surgeon’s skill, experience, and judgment. A highly experienced rhinoplasty surgeon can produce excellent results through either approach. What matters most is choosing a surgeon with specific, substantial rhinoplasty experience and the artistic sensibility to deliver a result that is natural, balanced, and harmonious with your overall facial features.
